Waiver Form

You have read, understood and agreed with the following:
 

A) YOUR PHYSICAL CONDITION AND SAFETY
1) It is your responsibility to consult a physician regarding your ability to participate in the physical activities and/or use of beeteeyogee's services.
2) You will progress at your own pace and you understand your physical limitations so you are sufficiently self-aware to stop physical activity before you become ill or injured.

3) If you are pregnant, or become pregnant, or are postnatal, you undertake that you are participating in yoga with your doctor’s full approval.

4) You shall not use any of beeteeyogee's services, or participate in any of the physical activities provided by beeteeyogee whilst suffering from any infections or contagious illness, disease or any other ailment including but not limited to open cuts, abrasions, open sores or minor infections, where there is any risk that such use or participation in such condition may be detrimental to the health, safety, comfort or physical condition of users of beeteeyogee and that you will bring any such matters to the attention of the staff of beeteeyogee if in any doubt.

5) You shall not participate in or use any of beeteeyogee's services if you are under the influence of drugs or alcohol.

 

B) LIMITATION OF LIABILITY

1 )You hereby acknowledge that you understand and voluntarily accept the risks of injury involved in the physical activities with beeteeyogee.

2) To the fullest extent permitted by applicable law, Bao Tran Tran, beeteeyogee, Beewell and its employees, teachers, contractors, agents, representative shall not be liable for any loss or damage to property or in respect of any bodily injuries, illness or death of you, your guests and/or other persons including third parties within the facilities, howsoever caused, aggravated or incurred arising out of or in any way connected with but not limited to any behaviour of you or your guests, the use of beeteeyogee's services.

3) You hereby knowingly, voluntarily, and expressly waive any claim you may have against Bao Tran Tran, beeteeyogee, Beewell and its teachers for injuries or damages that you may sustain as a result in participating in the yoga classes. By filling up this form, you acknowledge and agree that you shall not now, or at any time in the future, bring any legal action against Bao Tran Tran, beeteeyogee and/or any other person who may teach at beeteeyogee; and that this waiver is binding on you, your heirs, your spouse, your children, your legal representatives, your successors, and your assigns.

4) If you are under 18, you must have a parent’s or legal guardian's consent.

5) By filling up this form you are binding to this liability waiver from this day forth.

 

C) PHOTO RELEASE AND CONSENT

You grant consent that you may be photographed while practicing yoga with beeteeyogee and these photographs may appear in beeteeyogee's promotional materials unless otherwise specified. No person whose photograph is used will be identified by name, nor will any compensation be extended for such use.
